    • Job Requirements
    • American Heart Association certification is required for all direct patient care personnel.
    • Must include an image of the front of the card as well as an image of the back of the card.
    • Must be dated within 30 days of start.
    • Attach scanned online verification page with a machine-generated date indication when the verification was performed within the past 30 days.
    • 9 Panel completed within 30 days of start date, preferred within 14 days of start.
    • All engaged contractors must have page 27 signed form on file in Trinity VMS.
    • Standard i9 completed by the agency.
    • Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement must be signed.
    • Proof of vaccine, declination or exemption (with agency letterhead).
    • Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later.
